When he asked if its better to die as a good man or live as a monster and the way he came to the authority with no resistance and the look he gave when he quickly glanced at the bald doctor it was as if to say he was cured but he put on the act with dr Sheehan so Sheehan would think the role play was a failure. Reason for this is because he finally accepted reality and didn't want to go on as a monster so he preferred to die a good man, or at least have the brain surgery which would have either killed him or extracted the horrible memories. Living on in his past life as a Marshall was no longer an option because he knew it was a facade. The experiment of role play worked, that's why he had to pretend it didn't, so they'd lobotomies or kill him but either way it's an end to laeddis' pain.

If people are unsure, Cobb and Mal were stuck in Limbo and whilst Cobb knew this and accepted this, Mal began to believe that this was her reality. This is signified when she locks the totem in the safe and its not spinning. She is putting the idea in her head that she is in reality. Cobb knew she did this and went into her subconscious (the safe) to try and make her realise that she is in fact in a dream and needs to wake up. This is signified when he spins the totem and locks it in the safe, her subconscious. However, what Cobb didnt realise is that what happens in limbo then progresses up through each level into reality. So when they were in fact in reality after they killed them selves in limbo, Mal still believed that she was dreaming which is why she killed herself. This is why she was so sure that she was still dreaming but in fact they were both in reality. And this is how Cobb then realised that this was possible to do to Fisher, as he did it to Mal.
